Why did the Suntek-8500 voltage regulator burn out?

I have three phases. Three stabilizers Santek-8500 are installed. I combined input and output zero. Could this affect the failure of one of the three? However, they worked perfectly for a whole year. The connection diagram is as per the fact of the manufacturer.

The circuit seems to be correct! If your connection would be wrong - they would not work perfectly for a year, but would burn out immediately. That is, this could not lead to the fact that after a year one of them burned down. Perhaps there was a surge overvoltage (surge) in the network, as a result of a thunderstorm or any other problems. SPD could protect against this.
